Billing & Insurance

Health insurance is filed as a courtesy by our staff. We ask that patients please provide current and accurate information regarding insurance coverage. We are participating providers for Medicare, Medicaid and various HMOs and PPOs. Your health insurance may  require authorization for office visits, the authorization must be obtained before your visit.  All co-payments are expected at the time of service. Any outstanding balance that remains after payment from the insurance company is due within 30 days. We accept credit cards, cash and checks.

This is a listing of the insurance plans with which we are currently in network. If patients are unsure of insurance policy features, they should contact their insurance company before the first office visit. This list is subject to change.
